USPS Form 1583

USPS Form 1583 or the Application for Delivery of Mail Through Agent, is a document that authorizes a Commercial Mail Receiving Agency (CMRA) to receive and handle postal mail on behalf of a renter.


Any person or company renting a P.O. Box or a virtual mailbox directly through the post office or a mailing address from an approved third-party business location (called a CMRA) must fill this out. 


It is kept on file by the USPS United States Postal Service so they can keep track of the identity of those who use an address if a lousy actor uses it for an illegal purpose.

It is important that the form is filled out by the renter correctly, the correct IDs are presented & stated on the form, and also that the form is legally notarized.
