Acceptable Primary Forms of Photo Identification per Product and Service
U.S. Government I.D.: U.S. Government I.D. may be federal-, state-, or tribal-issued. Customers may provide the following:
State-issued driver’s license
Non-driver’s identification card
U.S. Armed Forces card
Uniformed Service ID card
U.S. permanent resident or other identification card issued by U.S. Citizenship and Immigration Services
U.S. certificate of citizenship or naturalization
Identification card issued by a federally or state-recognized tribal nation (tribal identification card)
Passport: This includes a U.S. passport, U.S. passport card, or foreign passport.
Matricula Consular (Mexico): A Matricula Consular card is an identification card issued by the Government of Mexico through its consulate offices to Mexican nationals residing outside Mexico.
NEXUS (Canada): NEXUS is a joint Canada Border Services Agency and U.S. Customs and Border Protection operated trusted traveler and expedited border control program. Customers may provide a NEXUS card as a form of identification for money orders. To be accepted, the NEXUS card must contain an identification number.
U.S. University Identification: For some retail products and services specified in Exhibit 10.3 customers may provide a public or private U.S. university identification card as an acceptable form of photo identification.

Unacceptable Forms of Identification
The Postal Service does not accept the following as either sufficient proof of identity or validation of an address; they are unacceptable as primary and secondary forms of identification:
Social Security cards
Birth certificates
Credit cards
Corporate identification card
Other similar items
Utility bill
When you fill out USPS Form 1583 to set up a virtual mailbox, Section 8 is where you give your photo ID information. This proves your identity so your mailbox can be approved.
8a. Applicant’s Name - write your full name exactly as it appears on your ID.
8b. Applicant’s ID Number - enter the number from your ID (driver’s license, passport, etc.).
8c. Issuing Entity - write the name of the agency that issued your ID (e.g., “California DMV” or “U.S. Department of State”).
8d. Expiration Date on the ID - write the expiration date listed on your ID.
8e. Photo ID Type - check the box for the type of ID you’re using (driver’s license, passport, military ID, green card, etc.).
Reminder:
Always include a clear copy of your IDs with your form. If applying online, make sure photos or scans are easy to read.
