Where and How to Sign Your USPS FORM 1583 (Section 13)



You’ll find Section 13 near the bottom of USPS Form 1583, typically labeled “Signature of Applicant”


Your signature must be witnessed by an authorized person, and you have two options for how to do this:

  1. Online Notary
    Log in to your mailbox account and choose the online notarization option. You’ll connect with a certified notary via video call, present your identification, and sign the form digitally while the notary witnesses it.

  2. Mail Center / In-Person Verification
    Visit your mail center or CMRA location in person. Bring your completed form and valid identification. The staff will verify your identity, watch you sign the form, and confirm the date of your signature.
