What Photo IDs You Need for USPS Form 1583 (Section 8)



When you fill out USPS Form 1583 to set up a virtual mailbox, Section 8 is where you give your photo ID information. This proves your identity so your mailbox can be approved.



  • 8a. Applicant’s Name - write your full name exactly as it appears on your ID.

  • 8b. Applicant’s ID Number - enter the number from your ID (driver’s license, passport, etc.).

  • 8c. Issuing Entity - write the name of the agency that issued your ID (e.g., “California DMV” or “U.S. Department of State”).

  • 8d. Expiration Date on the ID - write the expiration date listed on your ID.

  • 8e. Photo ID Type - check the box for the type of ID you’re using (driver’s license, passport, military ID, green card, etc.).


Reminder:

Always include a clear copy of your IDs with your form. If applying online, make sure photos or scans are easy to read.
